We truly appreciate our existing patients for helping our practice grow by continually referring their families and friends. That is by far the best compliment any practice can expect from their patients.

On your first appointment, you will be asked to complete medical questionnaire, which can be downloaded here or collected from the reception. Patients complete and sign a medical questionnaire on a regular basis to ensure that we have up to date information and can prove that we have.

Every patient has to sign blue form G17 given from the reception. This is a legal requirement for accepting treatment under the NHS.

To keep your registration up to date you need to see your dentist every six months.
